Cycle-day timing

Reproductive hormones swing through the month, so the day you test changes what the number means. Day 1 is the first day of your period.

  • FSH, LH and estradiol: day 3 for basic fertility testing, according to Cleveland Clinic.
  • Progesterone: day 21 of a 28-day cycle, or about 7 days before your next period, to check whether you ovulated. With longer or irregular cycles, count back from your expected period.
  • Testosterone and cortisol: in the morning, at the same time each test.
  • Hormonal contraception changes these hormones, so ask your clinician whether testing makes sense while you are on it.

The chart is why cycle day matters: an FSH of 10 IU/L is ordinary in the follicular phase but above ARUP's luteal range, and after menopause the whole range sits several times higher. FSH also swings from month to month in perimenopause, which is why doctors lean on symptoms and cycle history.

Thyroid

Thyroid symptoms (tiredness, weight change, hair thinning, cycle changes) overlap with many other things. A TSH alone is the usual screen. A fuller picture adds free T4, free T3 and TPO antibodies, which point to autoimmune thyroid disease. Ferritin and iron

Ferritin reflects your iron stores. MedlinePlus lists heavy menstrual bleeding and pregnancy among the reasons to check iron before symptoms appear. Vitamin D

The NIH Office of Dietary Supplements uses these serum 25(OH)D levels: below 12 ng/mL (30 nmol/L) is associated with deficiency, 12 to 20 ng/mL (30 to 50 nmol/L) may be inadequate for some people, 20 ng/mL (50 nmol/L) or more is adequate for most, and above 50 ng/mL (125 nmol/L) may cause harm. Heart and metabolic markers

Heart risk in women is often checked late. The 2018 AHA/ACC cholesterol guideline lists premature menopause (before 40) and pregnancy conditions such as preeclampsia among the risk enhancers doctors should consider, alongside ApoB of 130 mg/dL or more and Lp(a) of 50 mg/dL (125 nmol/L) or more. PCOS-type questions

If you have irregular periods, acne or excess hair growth, a clinician will usually look at androgens, pituitary hormones and blood sugar. How to prepare for a women's blood test at home

  • Write down your cycle day and whether you use hormonal contraception, and keep the note with your results.
  • Collect in the morning if the panel includes testosterone or cortisol, and at the same time on every repeat.
  • Wait if you are ill. Infection raises hs-CRP and can raise ferritin, which would hide low iron stores.
  • Pregnancy changes many reference ranges. If you are or might be pregnant, talk to your clinician before relying on a home panel.

Worked example: a 32-day cycle and two hormone kits

Take Ana, 34, a hypothetical reader with a regular 32-day cycle who wants to know whether she is ovulating and how her baseline hormones look.
