Two caveats. Counts include calculated ratios on some services (SiPhox counts ratios like ApoB:ApoA1), so a lower cost per marker is not always more information. And the cheapest per marker is not the cheapest way to get the few markers you care about, especially if you want them more than twice a year. Why prices vary so much

Most of the price difference between services is not the lab chemistry. It comes from four things:

• How the blood is collected. A lab visit uses a phlebotomist the lab already pays for. An at-home kit has to include collection hardware, a cold pack, an insulated mailer and overnight return shipping, which is why SiPhox charges $25 for shipping alone. A mobile phlebotomist coming to your home costs $79 to $119 extra.
• How many markers share one sample. Adding a marker to a sample that is already in the analyzer costs the lab little. That is why big panels like Function Health's work out at about $2 a marker while single tests cost $39 or more.
• Physician oversight. Direct-to-consumer lab orders still need a clinician to sign off. Quest shows this as a $6 fee; others build it into the price.
• Software and support. Memberships pay for dashboards, trend tracking, coaching and apps. You may or may not value those.

So the fair question is not "which test is cheapest" but "which test gives me the markers I want, collected the way I want, as often as I want, for the least money."

Is an at home blood test covered by insurance?

Usually not. Direct-to-consumer tests are sold as cash-pay: SiPhox says it does not accept insurance, and Quest says tests bought on questhealth.com are for people who want to pay directly and not seek reimbursement from their insurer. Function Health, Superpower and Everlywell 360 are also self-pay memberships.

What insurance does cover is lab work your doctor orders as part of your care. Under the ACA, plans must cover some screening blood tests at no cost in network, including cholesterol screening for adults of certain ages or higher risk, type 2 diabetes screening for adults 40 to 70 who are overweight or obese, hepatitis C screening for adults 18 to 79 and HIV screening for ages 15 to 65. If you only need the basics, asking for them at your annual physical is the cheapest route. The home services are for markers doctors rarely order (ApoB, Lp(a), a full hormone panel) or for testing more often.

HSA and FSA

• Physician fees. Quest adds a $6 physician service fee per order.
