VLDL cholesterol normal range: levels, calculation and risk

A normal VLDL cholesterol is under 30 mg/dL, and 30 mg/dL or more is high, according to Cleveland Clinic. Labs rarely measure VLDL directly: they estimate it as triglycerides divided by 5, so a high VLDL is really a high triglyceride result.

VLDL is a cholesterol measure, so it converts to mmol/L the same way as other cholesterol values: divide mg/dL by 38.67.

What VLDL is

Very-low-density lipoprotein is a particle made by the liver to carry triglycerides, the fat your body stores and burns, out to muscle and fat tissue. As it unloads triglycerides, VLDL shrinks into IDL and then LDL. Each VLDL particle carries one ApoB molecule, and Cleveland Clinic counts VLDL as a "bad" form of cholesterol because, in excess, it contributes to plaque buildup in the arteries. That is why VLDL is part of non-HDL cholesterol.

How VLDL is calculated

A standard lipid panel measures total cholesterol, HDL and triglycerides. The Friedewald equation, published in 1972, estimates the rest: VLDL = triglycerides / 5 and LDL = total cholesterol minus HDL minus VLDL. Cleveland Clinic notes that triglycerides are generally five times the VLDL level, but that the formula may not be accurate with severe hypertriglyceridemia, when other methods are used.

Because it is calculated, VLDL adds little beyond the triglyceride number itself. Cleveland Clinic points out that providers may not discuss VLDL directly and that it is not the direct target of treatment.

What high VLDL means

A VLDL of 30 mg/dL or more means triglycerides of about 150 mg/dL or more. Cleveland Clinic says high VLDL raises the risk of cardiovascular disease. Common reasons triglycerides and VLDL run high:

  • Excess weight, especially around the waist, and insulin resistance or type 2 diabetes
  • Diets high in sugar, refined carbohydrates and alcohol
  • Not fasting before the test, since triglycerides rise after a meal
  • An underactive thyroid, kidney disease and some medicines
  • Inherited lipid disorders

The 2026 ACC/AHA guideline notes ApoB testing is particularly useful when triglycerides are over 200 mg/dL, because that is when cholesterol-based estimates are most likely to miss particle-driven risk.

What lowers VLDL

Anything that lowers triglycerides lowers VLDL: weight loss if you carry extra weight, regular activity, cutting added sugar and alcohol, and managing blood sugar. Worked example

A 41-year-old has fasting triglycerides of 220 mg/dL, total cholesterol 210 and HDL 42. VLDL is 220 / 5 = 44 mg/dL, above the 30 limit. LDL is 210 minus 42 minus 44 = 124 mg/dL, and non-HDL is 168. His triglyceride:HDL ratio is 220 / 42 = 5.2. After four months of less alcohol and fewer refined carbs, triglycerides fall to 130: VLDL drops to 26, back under 30, and non-HDL falls by the same 18 points.

How to prepare for the test

  • Fast 9 to 12 hours. MedlinePlus says you may need to fast that long for a cholesterol test. Because VLDL is calculated from triglycerides, which rise after eating, a non-fasting sample can overstate it.
  • Skip alcohol the evening before. Alcohol raises triglycerides, and so VLDL.
  • Read triglycerides, not just VLDL. VLDL is triglycerides divided by 5, so the triglyceride number carries the same information and is what guidelines use.
  • Recheck after changes. Triglycerides respond to diet and alcohol within weeks, so a retest after 2 to 3 months shows progress.
