Cortisol test at home: morning blood cortisol or a 4-point saliva test?

Quick answer

Cortisol follows a daily rhythm: it peaks shortly after you wake and falls to its lowest at night. A cortisol test at home either catches one point on that curve with a morning blood sample, or maps the curve with saliva samples at four times of day. Labcorp's reference range is 6.2 to 19.4 mcg/dL in the morning and 2.3 to 11.9 mcg/dL in the afternoon, which shows how much the time of the draw matters.

Blood vs saliva cortisol

Estrogen, including the pill, raises the protein that carries cortisol in blood, so total blood cortisol reads higher on it; MedlinePlus lists birth control among medicines that can affect results. Saliva measures the free fraction and avoids that problem, which is one reason it is used for night-time testing. More on the hormone itself in our cortisol guide.

Normal cortisol ranges

A blood draw at 1pm judged against a morning range will look low when it is normal, so time your sample. SiPhox's EasyDraw instructions do not set a collection time, so if cortisol matters to you, collect before 10am and at a similar time on every test. Reading cortisol with DHEA-S and the ratios

SiPhox reports two calculated cortisol ratios. They are worked out from markers already on the panel, not measured separately, and we found no clinical guideline that sets a normal range for either, so treat them as personal trends rather than as results to compare with other people.

Both ratios carry cortisol's timing problem with them. Testosterone also peaks in the morning, which is why MedlinePlus and most labs ask for a morning sample for it too, so a morning draw keeps both halves of the ratio on the same part of their daily curves. A ratio from a 9am kit and one from a 3pm kit cannot be compared. Read the underlying numbers first: a ratio that changes because cortisol fell from 18 to 9 mcg/dL between a morning and an afternoon draw is telling you about the clock, not your stress.

How to time a home cortisol sample

With cortisol, the clock matters as much as the lab. The same person can land at the bottom of one range and the middle of another with nothing changed but the time.

For a blood cortisol you can compare over time:

1. Collect between 7am and 10am, the window Labcorp requires for its morning range, and at roughly the same clock time on every test.
2. Note when you woke up. Cortisol peaks shortly after waking, so a 9am sample after a 5am alarm and one after an 8:30am lie-in are not the same point on the curve.
3. Skip hard exercise beforehand. The EasyDraw instructions suggest light movement to warm up; keep it light, since exercise and stress both raise cortisol.
4. Write down anything unusual: a bad night, illness, a stressful morning, or starting or stopping birth control or other estrogen, which raises total blood cortisol.

For a saliva kit, follow the kit's four collection times exactly and do not eat, drink or brush your teeth for 30 minutes before each sample. A late-night sample taken an hour early can read high simply because cortisol had not fallen yet.

Which cortisol test to choose

What a home cortisol test cannot tell you

A single cortisol value, blood or saliva, cannot diagnose Cushing's syndrome or adrenal insufficiency. Endocrinologists confirm those with specific protocols: repeated late-night saliva, 24-hour urine or dexamethasone suppression tests for Cushing's, and an ACTH stimulation test for adrenal insufficiency. Symptoms such as unexplained weight gain with high blood pressure, or weakness, weight loss and dizziness, need a doctor rather than a kit.

Be wary of "adrenal fatigue". SiPhox's saliva test page uses the term, but the Endocrine Society says no scientific proof supports adrenal fatigue as a medical condition and that tests sold for it are not supported by good studies. A saliva curve can still be interesting for sleep and stress habits; it is not a diagnosis.

If a home result surprises you, take the number, the time you collected it and any medicines you take (especially steroid medicines and estrogen) to your doctor. MedlinePlus notes that many medicines can change cortisol results, so that context often explains an odd value faster than another test.
