Mayo also lists AMH for males, where it is highest in infancy (18 to 283 ng/mL under age 2) and under 13 ng/mL after age 12. AMH assays differ, so compare repeat results from the same lab.
What AMH measures
Anti-Mullerian hormone is made by the small, early-stage follicles in the ovaries, so its level reflects how many follicles, and therefore eggs, remain: the ovarian reserve. The ASRM explains that AMH declines as follicles are used up with age. Unlike FSH and estradiol, AMH stays fairly steady through the menstrual cycle, so it can be tested on any day, and MedlinePlus says no special preparation is needed.
What AMH does not tell you
The ASRM's 2020 committee opinion says ovarian reserve markers such as AMH are useful for predicting how many eggs IVF stimulation will retrieve, but are poor predictors of reproductive potential independent of age. The evidence behind that:
- In the EAGER trial of 1,202 women aged 18 to 40, women with AMH under 1 ng/mL had similar cumulative pregnancy rates to women with 1.0 to 3.5 ng/mL.
- In the Time to Conceive study of women aged 30 to 44 without known infertility, AMH under 0.7 ng/mL did not lower cumulative pregnancy rates after 6 or 12 cycles of trying (Steiner et al, JAMA 2017).
- For IVF, very low AMH matters: in more than 5,000 cycles with AMH under 0.16 ng/mL, 54% were canceled and the live birth rate was 9.5% per cycle started (ASRM).
MedlinePlus adds that a low AMH may mean menopause is approaching, but cannot predict when. In women under 40 with menopause symptoms, an AMH well below average for age may point to primary ovarian insufficiency, which needs a doctor.
High AMH and PCOS
Women with polycystic ovary syndrome have many small follicles, so their AMH is often high. The 2023 international PCOS guideline allows AMH as an alternative to ultrasound for the polycystic-ovary criterion, in adults only. MedlinePlus stresses that AMH alone cannot diagnose PCOS: doctors combine it with symptoms, androgen tests such as the free androgen index and medical history. See our PCOS blood test guide.
Worked example
A 36-year-old planning a family in two years gets an AMH of 1.1 ng/mL (1.1 x 7.14 = 7.9 pmol/L). That is inside Mayo's 0.15 to 7.5 range for 35 to 39, toward the lower end. On the Time to Conceive data, it does not mean she will struggle to conceive naturally. It does suggest fewer eggs would be retrieved if she chose egg freezing or IVF, which is the real use of the number, so it is worth discussing timing with a fertility doctor.
A 27-year-old with irregular periods and acne gets an AMH of 11 ng/mL (78.5 pmol/L), above Mayo's 9.9 upper limit for her age. With her symptoms, that fits PCOS, and a doctor would check androgens and rule out other causes before diagnosing it.
